Sample Signout of the Cellular Infiltrates Pattern

If no additional specific histologic features are identified, consider the following approach to signing the case out:

Cellular interstitial pneumonia (see comment).

Comment:  The biopsy shows extensive cellular interstitial infiltrates.  No additional specific histologic features to indicate an etiology are identified.  There is a broad differential diagnosis including subacute HP, cellular NSIP, CTD, infection, alveolar hemorrhage, drug reaction, aspiration, lymphoproliferative disease (including LIP), and as an idopathic entity.  The diagnosis of ILD requires a multidisciplinary approach.  Correlation with imaging studies, serology, microbiology studies, and clinical history is suggested.
