If no additional specific histologic features are identified, consider the following approach to signing the case out:
Acute and organizing lung injury with granulomas (see comment).
Comment: Granulomas in the setting of acute lung injury are concerning for an infectious process and correlation with final culture studies is suggested prior to immune suppression/immune modulation. Other possibilities include aspiration, drug reaction, and granulomatosis with polyangiitis (GPA). Necrosis would favor infection over GPA. Correlation with serologic studies is suggested.
