If no additional specific histologic features are identified, consider the following approach to signing the case out:

Acute and organizing lung injury with numerous hemosiderian laden macrophages (see comment).

Comment:  The biopsy shows evidence of acute lung injury along with numerous hemosiderian laden macrophages. Definitive capillarities is not appreciated. The changes are concerning for an alveolar hemorrhage process. Correlation with a history of hemoptysis is suggested. An extensive serologic work-up for serologic evidence of an alveolar hemorrhage syndrome is suggested. Other possibilities include ALI in a smoker (or previous smoker), ALI in a patient with congestive heart failure, and connective tissue disease.
