If no additional specific histologic features are identified, consider the following approach to signing the case out:
Acute and organizing lung injury with prominent neutrophils (see comment).
Comment: Neutrophils in the setting of acute lung injury is concerning for an infectious process. The presence of granulomas and necrosis would favor an infectious process. If there were numerous hemosiderian laden macrophages, this might support DAH with capillaritis. Correlation with final culture studies and serologic studies for ANCA associated vasculitis is suggested.
